Spinal cord constraint for re-irradiation after conventional RT

Patient had 30 Gy in 10 fractions to the same level two years ago. What cumulative cord constraint do you use for SBRT re-irradiation?

We use the published cumulative BED limits, stay conservative on the cord point dose and always discuss in our spine board. Time since first course matters a lot.

If there is epidural disease close to the cord, separation surgery first gives you much more room.
